Inflammatory myofibroblastic tumor is a rare intermediate-grade tumor. We herein report the case of an 81-year-old man with rectal ulceration and abnormal retroperitoneal soft tissue with a high serum level of IgG4. The administration of prednisolone reduced the retroperitoneal lesion; however, the rectal ulceration expanded. Surgical resection was performed. A histopathological examination revealed proliferating spindle cells accompanied by inflammatory cells and plasma cells. Liver metastasis emerged two months after surgical resection, and the histology of the proliferating spindle cells sampled by a fine-needle biopsy was similar to that of the rectal tissue. The patient ultimately died of inflammatory myofibroblastic tumor.

我々は電界砥粒制御技術を応用し，微小量の液滴を非接触に撹拌可能な技術を開発，酵素免疫測定法（ELISA）へ電界撹拌技術を導入し，迅速なELISA技術を提案している．本研究ではマイクロウェルプレート間における電界撹拌のばらつきを少なくし，撹拌の均一化を図るため，最適電界印加方法について検討した．結果として，一体型電極から分割型電極にすることによりプレート間の電界撹拌のばらつきを抑えることに成功した．

Clinical Significance of Molecular Diagnosis of Pilocytic Astrocytoma: A Case Report

This paper reports on a case of pilocytic astrocytoma (PA), for which a diagnosis by conventional pathological diagnosis was difficult but an accurate diagnosis was possible by a new molecular diagnostic method. A 13-year-old girl whose tumor developed by a headache that gradually worsened, and a well-demarcated T2-hyperintense lesion was found in the left cerebellum by a magnetic resonance imaging while the apparent diffusion coefficient value was also high. While the finding was a typical PA, histological features of PA were not found in the surgical specimen. An initial diagnosis was anaplastic astrocytoma (AA), and the final diagnosis through a central review was diffuse astrocytoma (DA). On the other hand, using MethylationEPIC (850 K) array, an analysis by a DNA methylation-based tumor classifier tool as reported by Capper et al. showed that this case belonged to a methylation class of PA. The copy number profile calculated from the methylation array data showed hints of BRAF/KIAA1549 fusion and no other chromosomal alterations, which also supported the molecular diagnosis. The patient was treated with local radiotherapy concomitant with temozolomide based on the initial pathological diagnosis during the consultation, but maintenance temozolomide therapy was not done according to the final molecular diagnosis. The tumor showed no recurrence for 20 months. In this case, the integrated diagnostic approach based on histological and molecular findings was clinically significant to select proper adjuvant treatment. It is crucial that the usefulness and robustness of this new molecular diagnostic method be validated further.

手術中に病気を視る迅速免疫染色入門

Immunohistochemistry (IHC) is the most common application of immunostaining. It is widely used in the disgnosis of abnormal cells such as those found in cancerous tumors. Intraoperative pathological diagnosis is also important in selecting suitable surgery for cancer patients. We performed intraoperative diagnosis of 830 cases in Akita University Hospital., with a novel method of rapid-immunohistochemistory (R-IHC) using an alternating current electric field. The results were very good, R-IHC detected an objective evidence that were undetected using conventional HE staining. So we can diagnose degree of malignancy, histological type of cancer, and lymph node metastasis more accurately.

In pathological diagnosis, it is necessary that the tumors are picked out by an operation, embedded in paraffin and sliced in thickness of around 2~5µm to prepare the slide glass. This slicing process is one of the important technologies that affect the accuracy of diagnosis. In this report, the mechanism of a micro slicing process of paraffin blocks was studied. As the temperature of paraffin blocks decrease, cutting force increases. Also, it doesn’t become zero where the setting depth of cut is zero due to tool edge roundness, and the model considering it was made.

術中迅速免疫組織染色を実現させる電界撹拌技術の開発

We aim to develop innovative stirring technology (electric field mixing (EFM)) applied abrasive control technique with AC electric field. This technique allows for stirring without contact by applying AC electric field to droplets. This technique was applied to intraoperative immunohistochemical staining. Under EFM showed antigen-antibody reaction time in immunohistochemical staining can be shortened from 90 minutes of conventional method to 10 minutes. In this study, EFM examined the mechanism which makes antigen-antibody reaction of an immunohistochemical staining quicken. As a result, we obtained that it has the mechanism which a reaction time of an immunohistochemical staining is shorten because antigens and antibodies improve to increase the frequencies of contact, and further antibodies disperse and becomes easy to react to antigens under EFM. compared with conventional method.
