<p>Here, we present a case of giant Brunnerʼs gland hyperplasia. Although it is often discovered as a submucosal lesion in the duodenum, a definitive diagnosis is challenging. Moreover, if the lesion is large or tends to increase in size, it may be difficult to distinguish it from a malignant neoplasm or it may result in an impaired passage or a complicated obstruction. A74-year-old woman had a 60-mm-sized submucosal tumor in the duodenal bulb, which was suspected to be Brunnerʼs gland hyperplasia. However, the patient did not wish to undergo treatment at that time, which led to the decision of just observing the patient. Regular follow-ups were performed; however, the lesion did not increase in size or cause any subjective symptoms. Eleven years later, the patient presented with epigastric pain. Contrast-enhanced CT and emergency esophagogastroduodenoscopy revealed the same lesion in the duodenal descending part. The lesion was resected via endoscopic submucosal dissection in the presence of a gastroenterology surgeon. Pathological examination revealed a 75×32×26 mm Brunnerʼs gland hyperplasia lesion. Since giant Brunnerʼs gland hyperplasia may cause obstruction or incarceration in the future, early prophylactic resection could be considered as a treatment option.</p>

<p>This paper reports the results of an investigation into the factors affecting the durability of the microtome blade used for sectioning pathology samples. Experiments were carried out to repeated sectioning of demineralized porcine ribs at a constant cutting thickness using an originally developed sectioning apparatus. Changes in cutting force, damage to the cutting edge and quality of the section due to repeated sectioning were measured, and the effects of microtome blade hardness, cutting edge angle and specimen characteristics on the durability of the microtome blade were investigated.</p>

<p>This paper reports the results of an investigation into the durability of microtome blades used for sectioning pathological samples. We conducted repeated sectioning experiments at a constant set thickness with automatic sectioning equipment we developed, using paraffin blocks embedded with porcine rib tissue as samples. We measured changes in cutting force and damage to the blade edge and examined the effects on the tissue quality of sections.</p>

<p>For pathological diagnosis of cancer and other diseases, it is necessary to embed diseased tissues in paraffin and cut them into sections that have thickness of several micrometers to get preparations for microscopic observation. In this study, we aim to improve and stabilize the quality of pathological sections. In this paper, we will report the results of experimental measurements of cutting force and thickness of pathological sections of various specimen-embedded paraffin blocks and the investigation of factors affecting the cutting force and thickness based on mechanical model.</p>

A 68-year-old man was referred to our hospital with a right-sided renal tumor identified by ultrasonography at the time of his medical check-up. Computed tomography revealed a well-circumscribed but distorted mass measuring 64×45×57 mm in the right kidney with para-aortic lymph node swelling. Laparoscopic right nephrectomy with para-aortic lymphadenectomy was performed. Histopathological diagnosis was mucinous tubular and spindle cell carcinoma (MTSCC) (pT3a) without lymph node metastasis (pN0). Postoperatively, metastases were identified in the lungs, and the vertebral and iliac bones. The patient was treated with axitinib. The lung nodule progressed and left sacrum metastases appeared even after treatment with axitinib. Therefore, nivolumab was administered as second-line treatment. The metastases in the lungs, as well as vertebral and iliac bone showed complete response to this therapy. MTSCC of the kidney is a rare low-grade renal cell carcinoma without any established systemic therapy for metastatic or unresectable lesions. We report a case of metastatic MTSCC in a patient who showed a favorable response to nivolumab treatment. This is the first report to describe successful treatment of metastatic MTSCC with anti-programmed cell death 1 antibody.

<p>Immunohistochemistry (IHC) is the most common application of immunostaining. It is widely used in the disgnosis of abnormal cells such as those found in cancerous tumors. Intraoperative pathological diagnosis is also important in selecting suitable surgery for cancer patients. We performed intraoperative diagnosis of 830 cases in Akita University Hospital., with a novel method of rapid-immunohistochemistory (R-IHC) using an alternating current electric field. The results were very good, R-IHC detected an objective evidence that were undetected using conventional HE staining. So we can diagnose degree of malignancy, histological type of cancer, and lymph node metastasis more accurately.</p>

In pathological diagnosis, it is necessary that the tumors are picked out by an operation, embedded in paraffin and sliced in thickness of around 2~5µm to prepare the slide glass. This slicing process is one of the important technologies that affect the accuracy of diagnosis. In this report, the mechanism of a micro slicing process of paraffin blocks was studied. As the temperature of paraffin blocks decrease, cutting force increases. Also, it doesn't become zero where the setting depth of cut is zero due to tool edge roundness, and the model considering it was made.

This technique allows for stirring without contact by applying AC electric field to droplets. This technique was applied to intraoperative immunohistochemical staining. Under EFM showed antigen-antibody reaction time in immunohistochemical staining can be shortened from 90 minutes of conventional method to 10 minutes. In this study, EFM examined the mechanism which makes antigen-antibody reaction of an immunohistochemical staining quicken. As a result, we obtained that it has the mechanism which a reaction time of an immunohistochemical staining is shorten because antigens and antibodies improve to increase the frequencies of contact, and further antibodies disperse and becomes easy to react to antigens under EFM. compared with conventional method.</p>

<p>By applied electric field mixing (EFM) showed antigen-antibody reaction time in an immunohistochemical staining can be shortened from 90 minutes of conventional method to 10 minutes. In this study, EFM examined the mechanism which makes antigen-antibody reaction of an immunohistochemical staining quicken. As a result, we obtained that it has the mechanism which a reaction time of an immunohistochemical staining is shorten because antigen and antibody improve to increase the frequencies of contact under EFM compared with conventional method. Further, we investigated the effect of AC electric field is applied on antibody. As a result, it found that by applied AC electric field to antibody liquid, antibody dispersed and stabilized, therefore, staining quality is improved.</p>

Therefore, this study aims to investigate the thinning mechanism in detail using a paraffin block. In this report, we report the effect of cutting conditions on the cutting data and morphology of thin sections measured using the tissue slicer we have developed.

We have developed an innovative stirring technology (electric field stirring technology (EST)) that enables stirring without contact by applying electric field abrasive grain control technology. In the previous study, we reported the characteristics of the EST and the shape of the electrode for efficient EST. In this study, the effect of stirring by EST was investigated. Using fluorescently labeled antibody, efficient diffusion and rapid antigen - antibody reaction were examined by microscopic observation and measurement using flow cytometer, respectively. As a result, it was found from the microscope observation that EST increased the antibody moving velocity by about 22 times compared to static condition. Furthermore, from the measurement results using flow cytometer, it was found that the reaction was accelerated 6 to 10 times by using miniaturization and electric field stirring. From these results, we have experimentally clarified the stirring effect.

<p>It is widely recognized that pathology is the most important factor for staging and selecting effective chemotherapy for patients with cancer. Immunohistochemistry (IHC) is a reliable screening method, but intra-operative diagnosis by frozen section with IHC is not possible because IHC takes approximately 6 hours. We recently developed a rapid-IHC method that makes use of an alternating current (AC) electric field to facilitate the antigen-antibody reaction, and reported its usefulness for detection of lung cancer and breast cancer. The antibody is mixed within microdroplets as the voltage is switched on and off at specific intervals. The resultant Coulomb force stirs the diluted solution on the sections, which increases the opportunity for contact. The rapid-IHC device reduces the time required for IHC as well as the amount of antibody required for analyses. The device may also be applied for in situ hybridization (ISH); however, ISH requires a warm temperature, such as 37℃, and a longer processing time than the rapid-IHC method. Consequently, evaporation can be a problem during ISH. The aim of the present study was to evaluate the utility and reliability of an oil cover and cover cap made of polyethylene terephthalate (PET) for preventing evaporation due to the AC electric field applied to facilitate the antigen-antibody reaction. We have shown that both the oil cover and PET cover cap prevented evaporation when using the rapid-IHC device.</p>
