<p>Polyurethanes are applied as artificial skin materials for medical simulators. There is a difference between human skin and artificial skin for simulators with respect to “puncture resistance.” Based on comments from medical doctors who perform puncture procedures in clinical sites, puncturing human skin is expressed as “low needle resistance,” whereas the artificial skin for simulators is expressed as “high needle resistance.” It is possible to practice medical procedures with a more realistic sensation if the artificial skin’s puncture resistance is brought closer to that of the human skin. In this study, sensory evaluations via needle suturing and quantitative evaluation via measuring devices were performed to clarify the relationship between the puncture resistance force and physical properties of three artificial skin samples for a suture training model made of polyurethanes. The composition and molecular structures of the materials were confirmed using Fourier transform infrared spectroscopy and nuclear magnetic resonance. The puncture resistance force measurements showed that Regina’s artificial skin: MODEL A was the lowest in three samples. Additionally, we discovered that low Young’s modulus and the amount of plasticizer contributed to the decreasing puncture resistance of MODEL A. This study proposes methods to combine sensory evaluation by humans and experimental data of the materials for the suture training model.</p>

An analysis of role behavior of the nurses who are Certified Diabetes Educator of Japan

<p>The purpose of this study was to analysis the role of the Nurses who are Certified Diabetes Educator of Japan (CDEJN), and a questionnaire was administered to 481 CDEJNs at 81 institutions who responded that they could cooperate with the study, and 199 responded to the questionnaire (41.4% response rate). The results of exploratory factor analysis revealed that the CDEJNs play six roles and behaviors: guidance for diabetic patients, guidance for complications and complication prevention, cooperation and collaboration in the diabetes guidance team, support for diabetes activities in the community, support for multidisciplinary guidance for diabetes care, reflection on diabetes nursing practice. Factor correlation coefficients ranged from 0.249 to 0.714, and Cronbach' s alpha coefficient for each factor was 0.939 for the entire CDEJN role behavior, and 0.822 to 0.915 within each factor. This study' s results suggest that more experience as a CDEJN is associated with more role behavior as a CDEJN.</p>

Current Status of Nurses in Support of Children with Parents with Cancer in Akita Prefecture

<p>Purpose: To clarify current status of nurses in support of children with parents with cancer in Akita Prefecture. Method: Questionnaire surveys were administered on nurses in Akita prefecture. Their experiences, educational backgrounds, and ability to recognize the need for support were examined. Descriptive statistics were used for the attributes, and chi-square test or Fisher's exact Test or Wilcoxon rank sum test was used to examine the relationship between the support experience and the attributes (<i>p</i><0.05). The contents of their open comments were then analyzed using a qualitative integration method. Results: Responses were obtained from 141 nurses at 10 facilities (collection rate: 43.9%). Of the 141 nurses, 135 (96%) recognized the necessity of support for children, and 28 (20%) already had experience supporting them. The contents of support were [direct support for children] [support for children through parents] [support through coordination and coordination with resources]. Most apparent difficulties in support were "Intervene for children" and "to have a connection with children". Conclusion: The results indicate that nurses recognize that children need support. However, they lack experience in providing such support. It is necessary to enhance support by learning support methods and sharing information with other occupations. Support from parents is important when children cannot be met. </p>

Vascular Access Teams for Ensuring Patient Safety in Japan

Safe insertion and management of central venous catheterization (CVC) is an important patient safety issue. Studies in several countries have shown that vascular access teams (VATs) are effective for maintaining patient safety. In this study, we reviewed VATs and their efficacy, and discussed the possibility of introducing VATs to Japan.

Although the precise efficacy of VATs remains unclear, some reports have indicated that they can decrease CVC complications and ensure good service quality. Introducing VATs to Japan might promote clinical practice reforms (work style reforms) and improve healthcare quality. Two problems must be overcome to introduce VAT to Japan: namely, the recruitment of vascular access specialists and education for novice team members. Referring to VAT recruitment in other countries can help in solving the recruitment problem. The education problem might be resolved via high quality education. To this end, we have created a new web-learning system that might help team members obtain expertise through step-by-step self-education.

Overall, we believe that introducing VAT will make significant contributions to healthcare quality and safety in Japan.
