  • The first nationwide website survey of the availability and costs of medical and non-medical oocyte cryopreservation in Japan

    RESEARCH QUESTION: How does the cost-related oocyte cryopreservation (OoC) vary by the facility in Japan, and what data is provided on the websites about OoC procedures? DESIGN: Website survey. The websites of all 621 facilities that provide assistive reproductive technology registered in Japan were surveyed in 2021. Data included the rates of explicit statements regarding the provision of OoC for only medical reasons (medical only group) or non-medical reasons (non-medical group). Based on whether or not facilities that perform OoC clearly stated the cost on their websites, we compared the costs of OoC and annual storage cost between medical only and non-medical groups. Furthermore, we examined the stated number of OoC procedures performed and their clinical outcomes. RESULTS: Of the 621 facilities, 146 (23.5%) clearly stated that they offer OoC on their websites. Of the 88 medical only groups and 58 non-medical groups, 24 (27.3%) and 42 (72.4%) clearly stated the OoC cost, and 27 (30.7%) and 44 (75.9%) clearly states the annual oocyte storage cost, respectively. The OoC costs were significantly higher for the non-medical group than in the medical group. In the medical only group, the annual storage cost remained almost the same regardless of the number of oocytes, while in the non-medical group, the annual storage cost was 2-3 times higher than in the medical only group. Only 16 facilities (16/146, 11.0%) had mentioned the number of OoC procedures, and five facilities (3.4%) provided information on the clinical outcomes after OoC. CONCLUSION: Costs related to OoC are higher for the non-medical group in Japan. In addition, the websites contain scant information on the costs and clinical outcomes of OoC.

  • Liquid biopsy of embryo culture medium for assessing human embryo viability using Raman spectroscopy

    This study investigated the potential of assessing human embryo viability through the analysis of the embryo culture medium used in assisted reproductive technology (ART) via Raman spectroscopy. Two types of culture media-one for early development (medium A) and one for recovery after vitrifying/warming (medium B)-were analyzed. Our results showed that protein concentrations were higher in medium A in which embryos reached the blastocyst stage compared to those in which embryos arrested before this stage. Moreover, the pH of medium A associated with higher-grade blastocysts tended to be more acidic than that associated with lower-grade blastocysts. Conversely, in medium B, pregnancy did not occur even with high-grade blastocysts when the medium was contaminated by residual vitrifying/warming agents or had slightly elevated component concentrations due to water evaporation. Furthermore, medium B tended to be slightly alkaline with higher-grade blastocysts, in contrast to medium A. The noncontact, noninvasive, and label-free Raman spectroscopic method for assessing human embryo viability based on culture media analysis may serve as a liquid biopsy in the near future. As ART has become an important treatment for infertility, this method, which may enhance its safety and efficacy, is expected to contribute to human welfare.

  • Analysis of Na+ concentration patterns in trophectoderm cells of mouse blastocysts using a dual-wavelength electrolyte indicator

    The developmental process of the mammalian blastocyst involves the intricate interplay of cellular and molecular mechanisms, including electrolyte dynamics within the trophectoderm (TE). We hypothesized that sodium (Na+) is actively transported from the TE into the blastocyst cavity, driving water influx and promoting blastocyst expansion. In this study, we investigated the dynamics of Na+ concentration in the TE of mouse embryos using sodium-binding benzofuran isophthalate (SBFI), a dual-wavelength Na+-sensitive fluorescent indicator. Observations revealed three distinct patterns of Na+ dynamics, each correlating with variations in blastocyst cross-sectional area and developmental outcomes. Embryos exhibiting an initial decrease followed by stabilization of Na+ concentration (Group A) demonstrated the highest rates of hatching, suggesting a relationship between Na+ flux and successful embryonic development. In contrast, embryos with transient increases (Group B) displayed reduced hatching rates and developmental progression. Further, the inhibition of Na+/K+-ATPase activity disrupted Na+ flux and blastocyst cavity expansion, emphasizing its critical role in blastocyst formation. This study highlights the potential of dual-wavelength imaging for elucidating electrolyte dynamics in preimplantation embryos and its implications for optimizing embryo culture systems in reproductive medicine.

  • Functional stability of the second mitotic spindle in blastomeres and its association with multinucleation repair

    OBJECTIVE: To study mitotic spindle morphology and chromosomal segregation in second mitosis. DESIGN: Live-cell imaging of deoxyribonucleic acid (DNA) and tubulin during the second mitosis of 21 freeze-thawed human two-pronuclear stage embryos was performed to analyze spindle morphology and chromosome segregation dynamics. Furthermore, chromosomal aneuploidy was assessed in all cells of the embryos that developed to the blastocyst stage. SUBJECTS: Twenty-one freeze-thawed human two-pronuclear embryos. EXPOSURE: We analyzed live-imaging videos of DNA and microtubules during the second division of 21 human 2-cell embryonic blastomeres. We further analyzed the association between the results of preimplantation genetic testing for aneuploidy of all cells in the observed embryos after development into blastocysts and the nuclear status at the early embryonic stage. MAIN OUTCOME MEASURES: Spindle morphology during the second mitosis, chromosomal segregation patterns, nuclear status of daughter blastomeres, and chromosomal aneuploidy of all cells in blastocysts derived from the observed embryos. RESULTS: Multinucleation rate in daughter nuclei was significantly lower after the second than after the first mitosis (22% vs. 56%). Neither defocusing of spindle poles-which was prominent in the first mitosis-nor chromosomal segregation abnormalities, such as misalignment of metaphase chromosomes and lagging chromosomes during chromosomal segregation, were observed. There was no association between the nuclear status at the 2- and 4-cell stages of the observed embryos that had progressed to the blastocyst stage and the preimplantation genetic testing for aneuploidy results of all cells in the blastocysts. CONCLUSION: Observations of mitotic spindle morphology and chromosomal segregation behavior revealed that second mitosis is less prone to chromosomal segregation errors than first mitosis. This suggests that segregation errors occurring during the first mitosis may be corrected during the second mitosis. Furthermore, there was no association between the nuclear status during early embryonic divisions and the chromosomal status of blastocyst cells, suggesting the presence of a mechanism that corrects chromosomal abnormalities during early development.
