Objective Although marital status has been demonstrated to affect patients' decisions to undergo active treatment for pancreatic cancer (PC), the role of other household cohabitants remains uncertain. This study aimed to examine whether the presence or absence of other household cohabitants, apart from spouses, affects the treatment decisions of patients with PC.

Methods and Patients A retrospective analysis of patients diagnosed with PC at the Akita University Hospital between January 2014 and December 2020 was conducted. Information about the patients' backgrounds, including the number of persons residing in their households, was gathered. A multivariate regression analysis was conducted to ascertain the factors associated with patients' decisions to undergo aggressive treatment, such as surgery, chemotherapy, and radiotherapy.

Results A total of 214 patients with histopathologically confirmed PC were enrolled in this study. Those who lived with others were more likely to receive aggressive treatment than those who lived alone (84.9% vs. 54.5%; p<0.01). The multivariate analysis, adjusted for patients' age, performance status, and marital status indicated that the patient's decision to undergo aggressive treatment was found to be positively associated with living with other household members with an odds ratio (95% confidence interval): 4.5 (1.02-19.9), p<0.05.

Conclusion Our findings indicate that the influence of cancer treatment decisions extends beyond that of spouses to encompass other household cohabitation members. By providing comprehensive social and emotional assistance to patients residing in isolation, it is possible to facilitate active treatment and enhance the prognosis for patients with PC.

Objective: Following the novel coronavirus disease 2019 (COVID-19) outbreak that began in 2020, contact between hospitalized patients and their families was significantly restricted to prevent infection. In May 2023, COVID-19 was classified as a Class 5 infectious disease by the Japanese Ministry of Health, Labour and Welfare. Although restrictions have gradually been lifted in some areas, at the time of writing this report, our hospital has not lifted all visit restrictions. Understandably, patients in this situation experience mental distress due to a lack of support from family, friends, or partners. Although remote visits have been introduced as an alternative to in-person visits, their effectiveness remains unclear. In this study, we report three cases in which we examine the impact of remote visits via electronic devices on patients who were unable to receive in-person visits.

Patients: Among the inpatients referred to our hospital’s palliative care team from April 2022 to March 2023, three who requested remote visits due to psychological distress caused by the inability to see their families were enrolled in this study.

Results: The degree of psychological distress in all cases reduced after conducting remote visits. However, cancer-related pain (in two patients) and postoperative wound pain (in one patient) showed no significant differences in severity. In one patient, anxiety was evoked following a remote visit. This was attributed to the patient’s realization of their isolation from their family.

Conclusion: For patients whose family members are unable to conduct in-person visits due to visit restrictions for various reasons, remote visits may help alleviate psychological distress. Since remote visits can cause adverse emotional reactions in some cases, monitoring the mental status during and after remote visits is necessary.

  • Grade 3 Pancreatic Neuroendocrine Tumor Refractory to Chemotherapy Successfully Treated with Peptide Receptor Radionuclide Therapy Leading to Conversion Surgery

Sakaki Kotaro, Chiba Mitsuru, Iijima Katsunori, Goto Takashi, Arita Junichi, Tsuda Hidehiko

    Internal Medicine ( 一般社団法人 日本内科学会 ) advpub ( 0 ) 679 - 685 2024

A 59-year-old woman was diagnosed with a pancreatic neuroendocrine tumor (P-NET; Grade 3, Ki67: 25%) with multiple liver and lymph node metastases and started chemotherapy with streptozosin (500 mg/m2/day) in combination with lanreotide acetate (120 mg). After six courses of (daily) streptozosin, the patient had progressive disease, as assessed by computed tomography (CT), and peptide receptor radionuclide therapy (PRRT) was started as second-line treatment. As PRRT was remarkably successful and the tumor shrank, surgery was performed to resect the primary pancreatic tumor, liver metastases, and lymph node metastases. CT evaluation performed six months after the surgery showed a complete response.

  • Peroral cholangioscopy for the evaluation of bile duct stricture in hepatocellular carcinoma on a preoperative examination

Chiba Mitsuru, Aokawa Masaki, Goto Takashi, Sato Wataru, Takahashi Kenichi, Minami Shinichiro, Iijima Katsunori

    Journal of Rural Medicine ( 一般社団法人 日本農村医学会 ) 19 ( 1 ) 44 - 48 2023

Objective: Bile duct tumor thrombosis in hepatocellular carcinoma (HCC) is a relatively rare event with a poor prognosis. Furthermore, bile duct tumor thrombus in HCC may be misdiagnosed when only imaging modalities are used. The efficiency of peroral cholangioscopy (POCS) in evaluating bile duct lesions has been reported.

Patients: We present three cases of HCC with bile duct strictures in which POCS was performed as a preoperative evaluation.

Results: In these three cases, diagnosing whether the lesion was a bile duct tumor thrombus on CT and endoscopic retrograde cholangiopancreatography was difficult. We performed POCS in three cases and were able to diagnose the presence of bile duct tumor thrombus of HCC, including differentiation from extrinsic compression of the bile duct.

Conclusion: POCS for HCC with bile duct features is useful for the preoperative diagnosis of bile duct tumor thrombus, especially in cases where the surgical procedure depends on the presence of bile duct tumor thrombus.
