Emotional Changes and Experiences of Parents with Cancer Following CLIMB® Program: A Pilot Study

<jats:p>When a parent raising a child develops cancer, he/she faces difficulties in continuing the parental role. For parents to face their own cancer treatment with peace of mind, supporting them in reducing challenges related to the parental role and promoting communication with their children is essential. This pilot study was designed to implement the Children’s Lives Include Moments of Bravery (CLIMB®) program for cancer parents and determine quality of life (QoL), psychological anxiety, and parents’ emotional changes/experiences. This study analyzed the changes in scores for QoL and psychological anxiety before and after participation in CLIMB®. The changes perceived by the parents were examined qualitatively and descriptively through semi-structured interviews. There were five participants (four mothers and one father). Their mean age was 48 ± 7.0 years. QoL increased from 63.4 ± 4.1 (63.0) to 69.2 ± 5.1 (70.0) points. On the subscale, emotional changes/experiences increased significantly from 16.6 ± 3.1 (16.0) to 20.4 ± 3.2 (20.0) points (p = 0.04). Psychological anxiety, measured using the State-Trait Anxiety Inventory (STAI), decreased for both state and trait anxiety (p = 0.04). After participating in CLIMB®, parents recognized their own hard work and felt less guilty about their children. In the parent-child relationship, parents and children were able to face cancer together and communicate their feelings to each other. It is essential to continue to intervene to promote communication between parents and children.</jats:p>

Children with a parent who has cancer express fears about cancer contagion, parental death and security of their life. CLIMB&reg; (Children&rsquo;s Lives Include Moments of Bravery) is a support program to improve children&rsquo;s ability to cope with their parent&rsquo;s cancer. This pilot study aims to describe the emotional impact of CLIMB&reg; on children with a parent who has cancer. The elementary school version of QOL (Quality of life), the SRS-C (Stress Response Scale for Children), Self Esteem, and satisfaction were evaluated.
Participants were seven children (three girls, four boys). All participants were satisfied with CLIMB&reg;. The QOL scores significantly increased from 79.9 (SD, 19.1; median, 86.7) points before the intervention to 85.1 (SD, 15.7; median, 90.0) points after the intervention, indicating an improvement in the QOL (p=0.046). Subscale was no significant difference according to sex. In the SRS-C scores no item showed significant differences in the pre- and post-intervention scores. The total score had decreased, indicating a decrease in the stress response. The boys showed a slight increase in the scores on the physical state subscale. The self-esteem scale scores were difference between the pre- and post-intervention scores increased significantly for the total score (p=0.028) and the subscales of &ldquo;self in relationships&rdquo; (p=0.042) and &ldquo;self-assertion and self-determination&rdquo; (p=0.038).
During CLIMB&reg;, children received accurate cancer knowledge; and valued sharing their feelings among others who are in the same situation. Children were highly satisfied with the program, although small changes were seen in QOL and stress. -

Challenge and hope for parents who have cancer

Parents who have cancer face particular problems in their relationships with children. This study aims to clarify 1) the current status of challenge/hope in parental cancer, 2) the factors related to challenge/hope, and 3) the relationship between challenge/hope and QOL/stress-coping ability. Cross-sectional national survey was conducted at designated cancer hospitals in Japan. Participants were undergoing cancer treatment and have children under 18 years old. The questionnaire included demographic data, QOL, ability to cope with stress, and challenge/hope. From 11 hospitals, 54 patients (response rate: 79.4%) participated. Majority of participants were female (72.2%) with the mean age of 39.3 ± 5.3 (SD). The total score was QOL (FACT-G: 50.4 ± 16.2), stress-coping ability (SOC: 46.7 ± 10.4). The main challenges were an inability to fulfill the parental role and children’s mental suffering due to loneliness. The main aspects of hope were the value of the children’s present self, being a parent, and strengthening family bonds. Gender and disease duration were significantly related with challenge/hope. Challenge was significantly related with QOL/stress-coping ability. Parents who have cancer derive hope from their relationship with their children, although they feel a gap between their ideal role/value as a parent and their current status.

The Short Questionnaire to Assess Health-enhancing Physical Activity (SQUASH) measures physical activity. This is a paper-and-pencil assessment. An electronic version of this scale, namely, the E-SQUASH, was developed to address the limitations of the paper-and-pencil version (e.g., missing data, time-consuming data entry processes, delay in sending feedback) and facilitate the handling of population data. This was a scale validation study. The test-retest reliability of the scale was examined the reproducibility of its scores. The consistency between the scores yielded by the E-SQUASH and paper-and-pencil version of the scale was examined. A total of 104 university students (response rate: 77%) participated. Regarding reproducibility, the two sets of total activity scores were strongly correlated (r = 0.68, p < 0.01), and the 95% CI (0.56–0.78) was adequate. There was a high level of consistency between the total activity scores that were yielded by the two versions of the scale (Intraclass correlation coefficient = 0.87). In this study, the reliability and validity of the E-SQUASH were established among university students. Further research is needed to convert this system into an application and translate its contents into different languages as well as across other age groups.

がん治療中の親が子どもとの関わりにおいて抱く困難と希望 親の語りに焦点を当てて

がん診療連携拠点病院で治療中の患者で18歳未満の子どもをもつ5名(男性1名、女性4名)に聞き取り調査を行い、子どもがいることで「困難を感じたこと」「希望を感じたこと」について聴取し、それぞれカテゴリー化した。結果、困難を感じたことは【自分のせいで子どもに負担を与えている】【治療によりいつも通りの生活ができなくなる】【子どもとの心の距離が離れてしまう】【子どもにとって良くない状況を想像して辛い】【子どもにとって良い親でいられない】【子どもの将来への不安】というカテゴリーに分けられた。希望を感じたことは【親としての在り方が治療に向かう気持ちを支える】【子どもを育てる責任感を感じる】【子どもと日常生活を過ごす】【子どもの成長を感じる】【子どもが自分の存在価値を取り戻してくれる】に分けられた。

秋田県におけるがんになった親をもつ子どもへの支援に対する看護師の現状

<p>【目的】がんになった親をもつ子どもへの支援に対する看護師の現状を明らかにする．【方法】平成28年12月～平成29年2月，秋田県内のがん看護に関わる看護師を対象に支援の認識や経験，学習背景等を質問紙調査した．属性は記述統計，支援経験と属性の関連はχ<sup>2</sup>検定，Fisherの正確確率検定等を用い（<i>p</i><0.05），自由記載は類似内容でカテゴリ分類をした．【結果】10施設141名（回収率：43.9％）のうち，支援が必要と思う者は135名（96％），支援経験者は28名（20％）であった．支援内容は［子どもへの直接的支援］［親を介した支援］［リソースへの連携調整による支援］，支援上の困難は「子どもへの介入」が最多で，「子どもと会えない」も挙がった．【結論】子どもへの支援経験者は少数のため，具体的な支援方法を学び，他職種との情報共有をすることで支援の充実を図り，子どもに会えない場合には親を通した支援が必要である．</p>

平成29年6〜7月にがんの親を持つ子どもへの支援に関する「アシスタント養成講座」を受講した医療者・医療系学生を対象に、がんの親をもつ子どもへの支援として、米国において開発されたプログラムであるChildren's Lives Include Moment of Braver(CLIMB)プログラムアシスタント養成講座の教育効果について検討した。3回の講座すべてを受講した12名(性別:男性1名、女性11名、職種:看護師7名、臨床心理士2名、医療系学生3名)に質問紙調査を実施し、全員から回答が得られた。質問紙調査の結果、実践現場への還元や、支援の認識と知識はCLIMBプログラム後3回受講後に向上し、プログラムの教育効果はあったと考えられた。今後は、支援の具体策に関する学習の継続が必要であり、支援の浸透のためには「子どもに関わる全ての職種への啓発」と「多職種のサポート体制を構築すること」が重要であることが示唆された。

整形外科下肢手術3ヵ月後における身体活動量の実態 SQUASH尺度による評価

2016年12月〜2018年6月にA病院整形外科外来を受診した下肢術後患者(人工股関節置換術後患者を除く)247例を対象としてSQUASH(健康的身体活動量尺度)による評価を行い、年齢や術式による違いについて検討し、以下の結論を得た。1)全人工膝関節群は年齢が高いものの、散歩や庭仕事などで活動量は維持されている。2)膝周囲骨切り群は、まだ働き盛りで活動量は元々多いため、仕事を継続している限り活動量は維持される。3)靱帯や半月板の手術を受けた群は、年齢が若くスポーツをしている者が多いため、将来的な関節保護の視点も重要である。

食道がん患者における退院後の経口栄養補助剤摂取状況の実態調査

A病院B病棟に入院歴があり、経口栄養補助剤(ONS)を処方された食道癌患者を対象に、退院後のONS摂取状況を明らかにするために質問紙調査を実施した。対象となった外来通院患者43名のうち、33名(男性26名、女性7名)から回答が得られた。ONSを導入された患者のうち、現在もONSを飲んでいるのは8名であった。退院後、ONS摂取を終了した時期としては、退院後1ヵ月が最も多く、13名、次いで1から3ヵ月が8名であった。ONSの説明は医師から受けたとの回答が16名と最も多かった。ONSは必要性について十分な説明がないと摂取しない患者がいることが分かった。退院後1ヵ月以内に摂取をやめる対象者が最も多く、体重維持、食事摂取量の増加、活動性の向上などが理由としてあげられた。入院中からONSの必要性や、摂取後の経過などを十分に説明する必要が重要であることが示された。特有な味や下痢を生じやすいなどの課題について、工夫を提示する必要性が示唆された。
