Introduction: Olfactory disorder develops relatively early in patients with neurodegenerative diseases such as Alzheimer's disease (AD). For verifying the usefulness of olfactory test as a screening tool for cognitive decline, we investigated the relationship between the olfactory identification test and Mini-Mental State Examination (MMSE) scores in community-dwelling participants of the Iwaki health promotion project. Since elderly people are not always conscious of olfactory dysfunction, we also examined the relationship of olfactory dysfunction awareness with cognitive decline.
Methods: We enrolled 844 participants (324 men and 520 women) in this study. Each participant was asked “How do you feel about the smell?", and they answered either “good", “average", or “poor". They also sniffed three odorants (India ink, cypress wood, and curry) in the odor stick identification test for Japanese. All the participants underwent MMSE. The MMSE scores were divided into two groups: the orientation and recall group and the remaining items group.
Results: In both men and women, the total MMSE scores significantly correlated with the simple olfactory identification test scores. When the MMSE scores were divided into two groups, the MMSE orientation and recall subset scores and the remaining 8 MMSE items' scores, a significant correlation was observed only between the MMSE orientation and recall subset and olfactory identification test scores in both men and women. Regarding olfactory awareness, the participants with MMSE scores ≤27 tended to have a dissociation between olfactory awareness and the ability to identify odors.
Discussion: Mild cognitive impairment (MCI) is recognized as a precursor to AD. About 10% patients with MCI shift to AD annually, but early detection and early response to the MCI stage with AD may delay onset. In this study, a significant correlation was found between the brief odor identification test and MMSE orientation and recall subset scores. Many participants having MMSE scores of ≤27 were unaware of their impaired odor identification ability. Further study is needed for establishing a convenient olfactory test to detect early cognitive decline. -

高齢者機能健診の参加者のうち,認知症やパーキンソン病,要介護認定者などではなく,外出頻度が多くない軽度認知障害(MCI)を有する83名を対象とし,介入群41名と対照群42名に割り付けた.介入群には6ヵ月間週2回の健康活動促進プログラムを提供し,対照群には2回の健康講座を提供した.介入群における48回の教室参加率は54.8%,対照群における2回の健康講座への参加率は68.3%であった.健康活動への参加回数を身体・知的・社会活動別に比較し,すべての活動において対照群に比べて介入群のほうで有意に多かった.介入前後における介入群と対照群の老年期うつ尺度(GDS)スコアを比較し,主効果ならびに交互作用は認めなかった.白質神経線維の結合性では,前視床放線における異方性比率(FA)が介入前は対照群と介入群で有意な違いがみられなかったが,6ヵ月の介入後には介入群に比べて対照群のFAが低かった.上縦束において介入前は対照群と介入群のFAに違いはみられなかったが,介入後には介入群に比べて対照群のほうで上縦束のFAが低かった.

<p>Mailing programs involve indirect supervision and are designed to enhance adherence to longterm exercise in older adults. However, it is unclear how such mailing programs help older adults to overcome barriers and adhere to an exercise program. The purpose of this study was to extract the effective factors of a mailing program based on mixed-method research. Community-dwelling adults aged 65 years or older (n = 78) participated in a 3-month exercise class that aimed to develop skills for behavior change, followed by a 12-month mailing program. During the 15-month study period, the participants were consistently advised to practice brisk walking, muscle strengthening, and stretching according to a self-determined schedule. They were also advised to set a monthly exercise goal, exercise daily, and record it in an exercise diary. At the end of each month, the participants mailed the exercise diary to instructors who provided feedback that included praise, suggestions, answers, recognition, sympathy, and encouragement. The instructors would return the diary to the participants along with an exercise newsletter consisting of scientific information on exercises, names of participants who had achieved their exercise goals, and comments from the participants themselves. The newsletter was designed to expand the participants' knowledge about exercise, to provide social support, and to strengthen self-efficacy. For qualitative evaluation, 18 participants with various exercise adherence levels were invited for focus-group interviews and their comments were recorded and analyzed. Fifteen factors related to the mailing program (e.g., satisfaction about record-keeping, self-reflection, information obtained, praise, and advice) were extracted. The participants' comments well reflected the goals needed for developing behavior change skills that were emphasized during the exercise class and the mailing program. For quantitative evaluation, a questionnaire was administered to understand the participants' perception of the mailing program, and 60 of them responded. The results indicated that daily recording in the exercise diary (95%) and gaining knowledge through the scientific information provided in the newsletter (93%) were perceived as effective factors of the mailing program. In conclusion, our qualitative evaluation of the effectiveness of the mailing program revealed 15 factors that were congruent with behavior change skills. Quantitative evaluation revealed that the majority of older adults considered the mailing program to be effective for enhancing their long-term adherence to exercise. Therefore, the mixed-method approach used in this study contributed to knowledge on factors that support exercise adherence among older adults.</p>

<b>Purpose</b>: The purpose of this study was to examine the influence of sharing exercise classes with spouses on marital relationship satisfaction in older adults.<br> <b>Methods</b>: Thirty-six older married couples and 39 older individuals participated in this study as couple and non-couple groups (CG and NCG, respectively). All participants attended an 8-week exercise class once a week and a home-based exercise program comprising walking and strength exercises. Marital relationship satisfaction was measured using questionnaires (quality marriage index (QMI) and satisfaction in marriage (SM)). We also assessed conversation time and frequency of self-disclosure and collective action with the spouse. Analysis of covariance adjusted for sex was conducted to compare the patterns of change in QMI and SM over the 8-week period between the 2 groups. Chi-squared test was also conducted to compare the proportion of participants who showed an increase in conversation time and frequency of self-disclosure and collective action with their spouse over the 8-week period between the 2 groups.<br> <b>Results</b>: The CG and NCG revealed no significant change in the QMI and SM after the intervention. Analysis of covariance showed that there were no significant interactions (time×groups) in QMI and SM (QMI: <i>P</i>=0.220, SM: <i>P</i>=0.110). The proportion of participants who showed an increased frequency of collective action related to hobby and exercise with their spouse was significantly higher in the CG than in the NCG (hobby: <i>P</i>=0.027, exercise: <i>P</i>=0.004), but there was no significant inter-group difference in the proportion of participants who showed an increase in conversation time and the frequency of self-disclosure.<br> <b>Conclusion</b>: These results suggest that sharing of exercise classes by older married couples can increase the degree of collective action in terms of exercise and hobby participation, but that its influence on marital relationship satisfaction, conversation and self-disclosure is limited. In order to re-examine the present findings, a better designed trial as well as an education program to learn communication skills for enjoyable exercise as a couple will be required.<br>
