<jats:p><jats:italic>Background</jats:italic>. Lymph node metastasis (LNM) is a relatively rare event in soft tissue sarcoma. An association between the timing of LNM detection and patient prognosis is presently unknown. <jats:italic>Patients and Methods</jats:italic>. We retrospectively analyzed the clinicopathological features of 33 patients with LNM between 2001 and 2015. Analysis of the timing of LNM diagnosis was grouped according to patients presenting LNM in either <8 months (the median time from primary tumor diagnosis to LNM) or ≥8 months after primary tumor diagnosis. <jats:italic>Results</jats:italic>. A relationship between the primary tumor size and the timing of the LNM was not significantly found (<jats:italic>Rs</jats:italic> = 0.0088, <mml:math xmlns:mml="http://www.w3.org/1998/Math/MathML" id="M1"><mml:mrow><mml:mi>p</mml:mi><mml:mo>=</mml:mo><mml:mn>0.96</mml:mn></mml:mrow></mml:math>). Sixteen patients had an LNM detection duration of <8 months, and 17 patients had a duration of ≥8 months. The 5-year survival for patients with an LNM detection duration of <8 months and ≥8 months was 19% and 71%, respectively (<mml:math xmlns:mml="http://www.w3.org/1998/Math/MathML" id="M2"><mml:mrow><mml:mi>p</mml:mi><mml:mo>=</mml:mo><mml:mn>0.0016</mml:mn></mml:mrow></mml:math>). There were 19 patients with pulmonary metastases. Among them, there were 13 patients with a duration of primary tumor diagnosis to LNM of <8 months and 6 with a duration of ≥8 months (<mml:math xmlns:mml="http://www.w3.org/1998/Math/MathML" id="M3"><mml:mrow><mml:mi>p</mml:mi><mml:mo>=</mml:mo><mml:mn>0.01</mml:mn></mml:mrow></mml:math>). <jats:italic>Conclusion</jats:italic>. Early LNM (<8 months) may predict poor prognosis in soft tissue sarcoma.</jats:p>

Treatment outcome of chest wall soft tissue sarcomas: Analysis of prognostic factors

<jats:title>Abstract</jats:title><jats:sec><jats:title>Background</jats:title><jats:p>Primary soft tissue sarcomas (STSs) involving the chest wall are uncommon. The aim of this study was to identify factors that influence the prognosis of patients with primary chest wall STS.</jats:p></jats:sec><jats:sec><jats:title>Methods</jats:title><jats:p>The records of 38 patients (23 men and 15 women) who were treated at our institutions during 2002 to 2018 were reviewed. The following variables were evaluated as potential prognostic factors: sex, tumor size, chemotherapy, and completeness of surgical margins. Multivariate analysis was conducted to identify predictors of overall survival (OS) and disease‐free survival (DFS).</jats:p></jats:sec><jats:sec><jats:title>Results</jats:title><jats:p>Of the 38 included patients, 5 had low‐grade tumors and 33 had high‐grade tumors. Five patients required chest wall reconstruction including rib resection. Thirty patients (79%) underwent R0 resection. The 5‐year OS and DFS rates were 45% and 27%, respectively. Local recurrence developed in 7 patients. Multivariate analysis identified tumor size (hazard ratio [HR]: 4.13; 95% confidence interval [CI]: 1.05‐16.24; <jats:italic>P</jats:italic> = .04) and R1/2 resection (HR: 3.92; 95% CI: 1.12‐13.66; <jats:italic>P</jats:italic> = .03) as predictors of OS.</jats:p></jats:sec><jats:sec><jats:title>Conclusions</jats:title><jats:p>Prognostic factors for survival included tumor size and completeness of surgical margins. Complete tumor excision is desirable, particularly in cases of early detection.</jats:p></jats:sec>

頚椎病的骨折を生じたリンパ管腫症の1例

<p>リンパ管腫症は小児慢性特定疾患に登録されているまれな疾患であり，治療法も確立されていない．今回われわれは，リンパ管腫症に頚椎の病的骨折を伴った1例を経験した．症例は17歳，野球練習中のスライディングを契機に受傷した．頚椎カラーによる保存加療で骨癒合を得ることが可能であり，症状の再燃も認めていない．しかしながら，今後も頚椎後弯や神経症状出現の有無について注意深い経過観察が必要である．</p>

BCG接種後に生じた肋骨骨髄炎の1例

<p>BCG接種後の副反応に関してはさまざまな報告がある．今回BCG接種後に右肋骨骨髄炎を生じた症例を報告する．症例は1歳9ヵ月，男児．生後5ヵ月でBCGを接種し，1歳8ヵ月で右前胸部の腫瘤を触知した．切開生検による病理組織学的検査にて，乾酪壊死と多核巨細胞の出現を伴う類上皮細胞肉芽腫を認め，BCG接種後骨髄炎と診断した．イソニアジドとリファンピシンを内服することにより治癒している．</p>
