<p><b>Objective</b>: The aim of the present study was to investigate the effects of shift work type on fatigue, both psychological and physical, and burnout among intensive care unit (ICU) nurses in Japan.</p><p><b>Methods</b>: From August to November 2021, a cross-sectional study on the type of shift work in Japan was conducted using an anonymous self-administered questionnaire survey (mailed). The survey contents included items on demographic characteristics, types of shift work in the ICU, the Japanese General Health Questionnaire-12, the Japanese Burnout Scale, and the “Jikaku-sho shirabe" work-related physical fatigue scale.</p><p><b>Results</b>: In total, 334 responses were received. A comparison of pre- and post-work “Jikaku-sho shirabe" scores indicated that 8-hour rotations (counterclockwise/backward) increased fatigue before the day shift, rather than after the day shift. On the Burnout Scale, 8-hour rotations (counterclockwise/backward) were significantly associated with high burnout, as indicated by a “decreased sense of personal accomplishment" (odds ratio = 4.79; 95% confidence interval: 1.57, 14.62).</p><p><b>Conclusions</b>: The 8-hour rotation (counterclockwise/backward) significantly increased burnout tendency and showed high physical fatigue before the day shift. Therefore, 8-hour rotations (counterclockwise/backward) may not be recommended for ICU nurses to continue to demonstrate advanced knowledge, rapid judgment, and nursing skills when dealing with patients. If 8-hour rotations (counterclockwise/backward) are chosen, to reduce “instability" before the day shift, sufficient consideration should be given to handover from the night shift, breaks during the shift, and follow-up.</p>

<p><b>目的:</b>看護職はゴム製品や薬剤等に触れる機会が多く,職業性アレルギーのリスクが高い集団といえる.そこで我々は今回,「看護職の職業性アレルギーと一次予防のための健康管理指針案」(以下,指針案とする)を作成し,臨床での活用の可能性と課題を明らかにすることを目的に質問紙調査を実施した.<b>対象と方法:</b>指針案の内容は文献等を参考にA. 職業性アレルギーの基礎知識 B. 看護職に多い職業性アレルギー疾患 C. 看護職における特定のアレルゲンによる職業性アレルギー D. 手湿疹と手のスキンケア E. 職業性アレルギーに対する健康管理とした.400床以上の同意が得られた80病院の看護管理者等各1名に質問紙調査を実施した.指針案で示した内容の理解度,方法を取り入れることに対する意見等を尋ねた.研究者所属の倫理委員会の承認を得て実施した.<b>結果:</b>30名から回答を得た.指針案で示した【職業性アレルギー】【看護職に多い職業性アレルギー】【手湿疹と手のスキンケア】については,理解できたと回答した者が70%を超え,だいたい理解できたと回答した者も合わせると100%であった.【作業管理】の内容を理解できた者は57%,取り入れたいと思った者は90%であった.取り入れたいと思ったができそうにないと回答した者は10%で,その理由は「全体周知と他職種も巻き込んだ取り組みが難しい」などであった.【作業環境管理】の内容を理解できた者は53%,取り入れたいと思った者は83%であった.取り入れたいと思ったができそうにないと回答した者は17%で,その理由は「アレルゲンのモニタリングを実際に行うのは難しい」「局所換気装置の設置は難しそう」などであった.<b>考察と結論:</b>指針案は,職業性アレルギーや健康管理方法の知識の提供に役立ち,臨床で活用可能であることが推察された.具体的に対策方法を取り入れるには,他職種を含め病院全体での理解と調整を図る必要があることが示唆された.</p>

<p>Purpose: To clarify how the introduction of a two-shift work at (intensive care unit; ICU) of A-Hospital affected nurse burnout and job satisfaction. Method: We conducted a Mann-Whitney U test and Spearman's rank correlation analysis on the relationship between nurse burnout, job satisfaction, and attributes of the nurse before and after the introduction of a two-shift work system in 36 nurses working at the ICU of A-Hospital. Result: There were no significant differences in all subscales of burnout before and after the introduction of the two-shift system, and the values after the introduction of the two-shift system were significantly higher for all factors of job satisfaction than before. After the introduction of the two-shift system, a moderate negative correlation (r = -0.50) was found with "workable work environment" and age, and a moderate negative correlation (r = -0.54) was found with all years of clinical experience. Discussion and Conclusion: The introduction of the two-shift work system suggested that it could improve job satisfaction. In addition, it is necessary to give sufficient explanation and consideration to nurses with many years of experience when introducing a two-shift work system in ICUs. </p>
